Partnership for Community Supports is a non-profit organization established to provide Supports Coordination.  Our goal is to assist individuals and their families in locating needed supports and services enabling the individual to have a full and meaningful life.  We believe that this can be best achieved by forging a partnership between the individuals, families, providers, Supports Coordinator and the community. It is through people working together and embracing teamwork, that a powerful relationships can be developed. Supports & Service Coordination requires kindness, patience, skillful communication, computer expertise, resourcefulness, time management skills, the ability to network on behalf of individuals, knowledge of many provider and community resources. A Supports & Service Coordinator facilitates team meetings designed to address the needs of individuals and their families in living everyday lives.
